With respect to patients who have had medical services from Sussex Partnership NHS Foundation Trust, for the period 1 January 2020 to 31 October 2025 please confirm:

1. The total number of requests by patients for their own personal data under the Data Protection Act 2018 and UK General Data Protection Regulation ('the legislation').

2. The total number of those requests where the 1 month time limit for responding under the legislation has been breached

3. The total number of those requests where the requirement under that legislation to inform a patient within 1 month that more than 1 month is required to provide the data has been breached.

4. The total number of times that the requirement under the legislation to provide the data within 3 months has been breached
