I am writing to advise you that following a search of our paper and electronic records, I have established that the information you requested is not held by the NHSBSA.

For questions 1 and 3 - No data is captured relating to the clinical indication a prescription is intended for. We hold data on which drugs have been prescribed, but not the condition it was prescribed to treat.
