Request

I am writing to make a request under the Freedom of Information Act 2000 / Environmental Information Regulations (as applicable). I seek the following information: In relation to Fairfield Dental Practice, 334 Stourbridge Road, Catshill, Bromsgrove, B61 9LJ (NHS contract holder): 1. The current Units of Dental Activity (UDA) contracted to that practice, and the associated annual contract value. 2. The number of UDAs actually delivered in the most recent full financial year, and the percentage of the contract fulfilled. 3. The total number of unique NHS patients seen by that practice in the most recent full financial year, and (if available) a breakdown by age group (children/adults). 4. Any data held on the geographical distribution of those patients (for example by local authority, MSOA, or LSOA), if available. 5. Whether the practice operates an NHS waiting list for new patients, and if so, how many patients are on it (as of the latest data). If any part of that request cannot be fulfilled because it falls outside data held, I request that you supply any related data you do hold (for instance, contract-level or commissioner-level statistics, or national summary data). Please also identify which part of the request is refused and under which exemption or reason. I would prefer to receive the information in electronic format (e.g. Excel or CSV), if possible.

I can confirm that the NHSBSA holds some of the information you have requested and a copy of the information and notes explaining it is below.

Please be aware, for Question 4, I have decided not to release the full details where the total number of patients falls below 5. This is because the patients could be identified, when combined with other information that may be in the public domain or reasonably available.

This information falls under the exemption in section 40 subsections 2 and 3A (a) of the Freedom of Information Act.

This is because it would breach the first data protection principle as: a) it is not fair to disclose patients personal details to the world and is likely to cause damage or distress. b) these details are not of sufficient interest to the public to warrant an intrusion into the privacy of the patients.

This report contains the details from FP17s processed for the period stated 2024/2025 at Contract 1009150000. The information within the report includes the data items and descriptions listed below. Activity by Foundation Dentists is excluded.

Reporting Year: The financial year which the activity relate to. Year-end methodology will include all activity data (including amendments) collected from FP17s scheduled in any of the fifteen schedule months from April to June, where the date of completion is on or between 1 April and 31 March of the year. For example 2019-20 will include all activity data (including amendments) collected from FP17s scheduled in any of the fifteen schedule months from April 2019 to June 2020, where the date of completion is on or between 1 April 2019 and 31 March 2020. Where FP17s do not have a date of completion, e.g. where the patient has failed to return to complete the treatment, the date of acceptance is used.
Contract Number: This is a unique 10 character number which identifies a contract

Principle Practice Address:
This is the Principle Practice Address of the contract .
UDA Performance Target: The contracted units of dental activity to be achieved for the year, as entered on CoMPASS by the PCO.
UDA Financial Value: The financial value associated with the UDA performance target for the financial year, as entered on CoMPASS by the PCO.
UDA Delivered:
The units of dental activity scheduled for the financial year to date for the period selected, relating to the financial year e.g. in December this will cover the scheduled months of April to December, excluding FD UDA for English contracts and and including UDA for Welsh contracts.
% UDA Delivered: UDA Delivered - UDA Carry Forward / UDA Performance Target. UDA Adjusted Delivered: UDA Delivered minus UDA Carry Forward plus NPP Total Credits & UDA Misc Credits

Unique Patient Count: This is a count of the unique patient identities on forms scheduled. A patient may have attended more than once in the time period and received different treatment, so the total figure has been adjusted for duplicate patient IDs. Patient identities are counted for all valid FP17 transactions – including amendment, deletion and withdrawn records. Therefore, it is possible for the count of patient identities to be greater than the count of FP17s.
Local Authority Name: The local authority name in which the patient is resident, irrespective of where treatment took place. Based on the home postcode recorded in the personal details section of each FP17 submitted, therefore is dependent on this information being included and accurate in the records. This is not dependent on where the patient received treatment, so can either be in or outside the PCO.

Adult/Child Description: Adult/Child Patients are defined based on the patients’ age at the date of acceptance. An adult is defined as a patient aged 18 or over at the date of acceptance, a child aged under 18.
10 Year Band: Age for the patient on the date of acceptance. Derived from patient date of birth and date of acceptance. Split in to 10 year groups.

Note: We do not hold data on whether the practice operates an NHS waiting list for new patients as that would be something held by the practice.
