This report contains the details from FP17s processed for the period stated in England & Wales. The information within the report includes the data items and descriptions listed below. Contract paid by NHSBSA.

Country Name – The Country where the contract is located.

Contract number - This is a unique 10 character number which identifies a contract

Latest Contract Type – Indicates the current type of contract as it pertains to the treatment rendered - Either GDS – General Dental Services, PDS – Personal Dental Services or PDS Plus – Personal Dental Services Plus

Latest Provider Name – Name of the current Contract Provider

Latest Contract Start Date - The start date of the contract, as recorded on CoMPASS. In this instance filtered to be less than or equal to 01/04/2017 – the date as requested

Latest Contract End Date – The end date of the contract, as recorded on CoMPASS – In this instance filtered to NULL as FOI request included only active contracts

Latest Principal Address Line 1 - The latest principle practice and correspondence address line 1 associated with the contract

Latest Principal Address Line 2 - The latest principle practice and correspondence address line 2 associated with the contract

Latest Principal Address Line 3 - The latest principle practice and correspondence address line 3 associated with the contract

Latest Principal Address Line 4 - The latest principle practice and correspondence address line 4 associated with the contract

Latest Principal Address Line 5 - The latest principle practice and correspondence address line 5 associated with the contract

Latest Principal Postcode - The latest principle practice and correspondence address postcode associated with the contract
