Question 2
Following a search of our paper and electronic records, I have established that the information you requested is not held by the NHS Business Services Authority. This is because we do not record reasonable adjustments that have been requested or granted.
Question 3
The FP92A is the application form to apply for a Medical Exemption certificate (MEDEX). This form is a controlled document and can only be issued and signed by a GP or medical representative who has access to applicable medical records.
A GP or medical representative will be able to assist with the completion of this form.
