Please provide a list of all dental practices across the UK that have failed in completing their UDA target during the period specified.

Please confirm in relation to these practices what their UDA target was, as well as their UDA value, how many UDAs they completed, how many UDAs they did not complete, what the clawback value is for each practice and when this is to be collected.

Please also provide a list of all of the dental practices seeking to return their NHS contracts, together with, contact names, numbers and email addresses

We will be in a position to publish by 30th September 2023, as the 28 day review will not be completed by 6th September 2023 (legal deadline)

Under Section 22 of the Act, we are not required to provide information in response to a request if it is scheduled to be published in the future. The information you requested is due to be published before the end of September 2023.
