‘I would like to request Summary of UK-issued S1 and S2 forms and EHIC, GHIC use, by EU Member State from 2004 to 2022. Could you please provide me the above information required for my bachelor dissertation at the Adam Mickiewicz university in the Department of Political Science and Journalism. Based on EU and UK agreement about reciprocal healthcare l would like to also obtain information about Polish citizens used NHS services based on that (EHIC) from 2004 to 2022

I would like to receive from you data covering period between 2004 (including) to 2022( including) stating how many : - S1 and S2 forms has been issued by UK to be used in Poland and each EU countries in that period of time - EHIC/GHIC cards have been issued by the UK to be used in Poland and other EU countries each of those years’

A copy of the information is attached. Please read the below notes to ensure correct understanding of the data.

Question 1 Please note the data prior to 2015 may be incomplete.

The S1 and S2 forms have had different names in the past. The S1 table shows the number of S1s issued. We have also included healthcare entitlement forms E106, E109, E120, E121, E122 which were used prior to the creation of the S1. The S2 table shows the number of S2s issued including their equivalent entitlement E112.

Question 2 Please note EHICs, UK EHICs and UK GHICs are not issued for use in an individual country, therefore the table provides figures of all issued EHICs, UK EHICs and UK GHICs.

UK GHICs replaced EHICs in 2021.

The figures provided start from 2005, as we do not hold data for 2004.

The data for EHIC provided from 2005 to 2012, may be lower than the actual figure due to data retention policy.
