2026-02-04: Description in data dictionary/schema for VACCINE_NAME changed from 'The text name of the vaccine product (pack) that was supplied and claimed for. Text may not correspond exactly to the NHS Dictionary of Medicines and Devices (dm+d)' to now read 'The text name of the vaccine product (pack) from which the dose was supplied. Text may not correspond exactly to the NHS Dictionary of Medicines and Devices (dm+d). Actual Medicinal Product Pack (AMPP) descriptions also include the pack size which may be greater than the number of doses supplied to a patient. For example a '10 unit dose pack' could be used to supply one unit dose to 10 patients.'
