I am conducting a 4 nations research survey of GPs and wish to email this to every GP practice in England (and devolved nations).

I would like to request the contact details for all currently registered GP practices in England. Their contact details MUST include the following details in Excel format please:

Name of the GP practice

GP registration number - ODS Code

Business address, premises Postcode - County and ICB

Official NHS shared email address of the GP practice (e.g. ****@nhs.net)

Under Section 21 of the FOIA we are not required to provide information in response to a request if it is already reasonably accessible to you.

Please note that we do not hold GP email addresses.
