1. A copy of the most recent guidance issued to dental practices on updating gender on a patients NHS Dental record from the NHS Dentist Business Authority Service and date this was issued

3. Any meeting minutes, EIA or other documents relating to any changes between the two versions. If this request is too broad, please narrow it down to any documents produced within the 3 months before the latest guidance was issued. If this is still too broad please disregard this part of the request and only answer the first 2 questions.

I am writing to advise you that following a search of our paper and electronic records, I have established that the information you requested is not held by the NHSBSA. The NHSBSA does not provide guidance to NHS Dentists on how to update their patient records.
