1) how many times has a form been sent through, each year for the last 5 years
2) in the last 3 years, how many times has the query form, sent by the form filler, included a match for the words:
a) "deepoll"
b) "unenrolled deedpoll"
c) "enrolled deedpoll"
3) does the PCSE have a a standard reply or set of guidance that they relay to GPs who ask questions about accepting unenrolled deed polls as evidence of a name change?
