I am writing to request information under the Freedom of Information Act 2000 regarding the NHS dental contract held by Eastbury Dental Practice (also known as Eastbury Dental Partnership) located at 99 Eastbury Road, Watford, Hertfordshire, WD19 4JP. Please provide the following information for the financial years 2024/2025 and 2025/2026 (to the latest available month):

  1. The contract number and commissioning organisation (ICB/region) associated with this practice.

  2. The number of Units of Dental Activity (UDAs) contracted for each year.

  3. The number of UDAs delivered for each year (and/or to the latest available reporting period for 2025/26).

  4. The percentage of the contracted UDAs delivered for each period.

  5. Any notes or records of contract under-delivery, clawback, or performance management action taken in relation to this practice during these periods.

If possible, please provide this data in CSV or Excel format.

To help identify the correct record, the practice is registered on the NHS website as: Eastbury Dental Partnership, 99 Eastbury Road, Watford, WD19 4JP. If any part of this request exceeds the appropriate cost limit, please advise how it could be refined.
